Madden 20 realistic sliders are based on members of the community complaining about the difficulty level of All-Madden. It's the hardest game mode and it's not meant to be easy. Yet, EA has done a poor job of creating difficulty. Players have posted issues on Reddit and the lack of control the sliders provide.

Instead of creating a more intelligent opponent, your own team tends to sabotage itself. Penalties, injuries, turnovers, lack of blocking, and defensive coverage is all below average no matter the team. And your opponent will run and pass all over you without problem.

Madden 20 Realistic Sliders: How to Set a Realistic Challenge

You can personally tweak with the sliders yourself trying out different functions and testing them in games, but this will take time. Instead, you can visit OperationSports.com where you can test different slider configurations

To start, you should set the computer setting "block" down to 35 and "QB Accuracy" to 40. You should also turn down "injury" to 40 as well. This should be a good starting point for you. Then you'll need to test the rest of your features.
